Job Details
- Under the direction of the Clinical Team Manager and within established organizational and departmental policies and procedures, responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice.
- The RN case manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, promotes and maintains health of individuals through direct care, teaching, counseling and appropriate rehabilitative measures.
Job Requirements
- Primary Source Verification of professional license and/or certification, as required for the position, within 30 days of start date and updated upon expiration.
- MA Conditional Licenses not accepted.
- Both NURSYS and BON are required for nursing licenses.
- Documentation of two measles vaccines, two mumps vaccine, and one rubella (German measles) vaccines or two MMR vaccines, or proof of immunity via blood test.
- Vaccine required during flu season Sept 15th - April 30th with no exemptions or waivers.
- Must have documentation of TB skin test screening within 3 months of start date and updated if >6 month break, or documentation of a negative QuantiFERON TB Test or T-spot within 3 months of start date and updated if >6 month break.
- Physician Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break.
- MVR check to be run by background check company within 45 days of start, required for all home care positions.
- MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break.
- MGB Massachusetts CORI background check ran within 30 days prior to start date and updated if >6 month break.
- Include past 7 years of work experience with all work experience and education including city & state.
- 2 recent references, minimum 1 Supervisor or Above, in Skillset.
- Required skills checklist for submission.
Schedule Information
- Monday – Friday 8am -4:30 pm with weekend rotation of 1 /4 weekend requirement.
Additional Details
- Fit Test for N95 respirator within 365 days to start date and updated annually.
- Driver's License/State ID with front of card and signature if applicable.
- MGB Badge Photo: color photo taken in past 6 months with plain background, no selfies or filters.
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions, updated if start date changes.
- Respirator Medical Clearance (RMC) Attestation required if fit test is required for skill.
- MGB Education Verification verified through background check company.
- MGB SSN Trace must be run with criminal check and list addresses/aliases for past 7 years.
- MGB National Sex Offender Check ran within 30 days of start and updated annually.
- MGB Exclusion Checks ran within 30 days of start and annually, including OIG, GSA, MassHealth & NH Medicaid exclusion lists.
- Candidate Submission Coversheet required for submission.
