Job Summary:

We are seeking a motivated and versatile Trainer to join our team and provide training on Enhanced Care Management and Community Support programs. The Trainer will be responsible for delivering engaging and effective training sessions to healthcare professionals, support staff, and community partners. The ideal candidate will be proactive, a quick learner, possess excellent communication skills, work well in a team, and be willing to travel to various locations.

Responsibilities:

  • Design and conduct comprehensive training programs on Enhanced Care Management and Community Support programs, ensuring alignment with program goals and audience needs.
  • Deliver interactive training sessions for healthcare professionals, support staff, and community partners, employing a variety of instructional techniques and tools.
  • Foster a positive learning environment, encouraging participant engagement, and facilitating knowledge transfer and skill development.
  • Collaborate with subject matter experts and managers to gather and update training materials, incorporating the latest guidelines and best practices.
  • Assess training needs and customize training sessions to address specific knowledge gaps and learning objectives of diverse audiences.
  • Evaluate the effectiveness of training programs through participant feedback, assessments, and performance evaluations, making adjustments as necessary.
  • Provide individual coaching and support to participants, reinforcing learning outcomes and assisting with program implementation.
  • Work collaboratively with management to enhance training resources, such as presentations, manuals, and online modules.
  • Stay informed about program updates, industry trends, and emerging practices in care management and community support to ensure up-to-date training content.
  • Maintain accurate training records, including attendance, participant evaluations, and program documentation.
  • Perform additional duties as assigned.

Education and Experience:

  • Bachelor's degree in a relevant field or equivalent experience in healthcare or training required.
  • Proven experience in delivering training programs, preferably in the healthcare or social services sector.
  • Strong knowledge and understanding of healthcare programs, including program guidelines, documentation requirements, and reporting procedures preferred.

Required Skills/Abilities:

  • Excellent verbal and written communication skills with the ability to effectively convey complex information to diverse audiences.
  • Proactive and self-directed, able to quickly grasp program details and adapt to changes.
  • Exceptional interpersonal skills, fostering collaboration and teamwork.
  • Proficient in training tools and technologies, including presentation software and learning management systems.
  • Ability to work independently, manage time effectively, and meet training objectives.
  • Demonstrated flexibility, adaptability, and a customer-focused approach.
  • Proficient in Microsoft Office Suite or related software.

Physical Requirements:

  • Willingness to travel to different locations to conduct training sessions.
  • Prolonged periods of sitting at an office desk on the computer.
  • Lifting: Able to lift up to 15lbs.

Pay range

$68,640—$75,000 USD
