York Hospital is seeking a *Home Care Physical Therapist* to join our *Home Care *Team!

*$10,000 SIGN ON AVAILABLE FOR FULL-TIME*

The Physical Therapist is responsible for providing physical therapy services to patients in their homes in accordance with a written Plan of Care approved by a physician. The Physical Therapist makes an initial evaluation and plans with patient, and physician for on-going physical therapy services. Initiates, reviews and implements the PT Care Plan for each patient based on the assessment and prescribed Plan of Care and revises when indicated.

*Requirements:*
* 1 year of Physical Therapy experience preferred
* Exceptional customer service and computer skills
* State of Maine PT License
* Basic Life Support Certification
* Valid driver's license
