This position is a Full Time/Day role at Gateway Plaza

The Virtual Integrated Behavioral Health team provides innovative behavioral health services through a virtual model. The team works collaboratively with outpatient health care providers. Team members adapt to the changing needs of this growing program.

DUTIES AND RESPONSIBILITIES

1. Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient's medical provider and, when appropriate, other mental health providers.

2. Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.

3. Provide patient education about common mental health and substance abuse disorders and the available treatment options.

4. Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.

5. Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.

6. Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.

7. Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.

8. Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.

9. Track patient follow up and clinical outcomes using a registry. Document encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.

10. Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.

11. Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.

12. Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).

13. Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.

Effectively communicates and collaborates with all members of the health care team to facilitate the episode of care so the patient receives services in a safe and timely manner, and the patient and family are provided all necessary resources and education. Interprets and communicates complex patient/family needs and role models caring practices to other caregivers. Assess patients' and families' psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, reaction to illness, and ability to cope. Counsels patients and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs. Provides mental health assessments of patients upon physician referral. Provides therapeutic intervention to facilitate complex discharge planning. Serves as the point person in child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence. Serves as liaison between Indiana University Health and state and county child and adult protective teams. Provides intervention in guardianship (temporary/permanent), foster care, adoption, and mental health level II placements. Provides specialized mental health services including, but not limited to, Crisis Intervention and Addiction Assessments and recommendations for psychiatric referral or community resources. Assists with dangerous and potentially dangerous situations and takes a leadership stance to provide clinical intervention. Supports those undergoing acute crisis which often results in the use of de-escalation techniques necessary to avoid episodes of physical violence. Utilizes effective principles of interviewing alleged victims, perpetrators, family members, and significant others. Provides intervention for end-of-life, guardianship (temporary/permanent), foster care, adoption, etc.

  • Master's Degree in Social Work required.
  • De-escalation certification required within 6 months.
  • Permanent LSW licensure required, LCSW licensure preferred.
  • 3-5 years experience preferred in one of the following areas (mental health, Department of Child and Family Services, adult protective services, Veteran's Administration, substance abuse).
  • Requires experience working with geographically and financially diverse patient populations.
  • Requires knowledge of the impact of the disease process and of human behavior to provide psychosocial assessments, short-term therapeutic counseling and crisis intervention to patients, families and staff.
  • Requires knowledge of state and county adult and child protective legal mandates.
  • Requires knowledge of the principles of growth and development over the life span.
  • Maintains principles, practices and techniques of crisis intervention and de-escalation techniques.
  • Develop and maintain strong working relationships with the community, medical teams, law enforcement, APS/CPS, schools, probation and other agencies as indicated.
  • Requires proficiency in Microsoft Excel and Word.
